It would spread between hearthlings through sharing of beds, hats, and other headwear.

Would debuff will, as you fall into desperation due to the terrible itch.

To treat it you could maybe rub different kinds of ointments to the head and comb regularly.

A fun little addition I'd say. It will add depth to the game and increase meaningfulness of player interactions.
Just imagine a village circulating lice around, very nice event. Much fun.

Its not just another wound but completely new dynamic mechanic. Lice infections in village would be so cool!

Having lice in a game is not a meaningful content. It's not even a content. Game already has an abundance of mechanics and systems that are not polished or up to date to the rest of the game.
And for the end, if there was a list of things that need to be added to the game, lice wouldn't even be on it.
