I propose an addiction mechanic.

Smoking once per day or so is fine and gives +1 charisma for a while.
If you smoke more frequently than that you get hit with an incurable addiction wound, where you have to keep smoking or suffer increasing stat decreases.
It would probably help create a market for it actually, as the addicts have to maintain a steady supply of pipeweed to suppress the debuff.

If you smoke lot. And then dont smoke for day(s) you will get big energy drain and huge random debuffs on attributes and stats.
So if you dont keep smoking you die. But if you keep smoking you get cancer and die
