I propose an addiction mechanic.
Smoking once per day or so is fine and gives +1 charisma for a while.
If you smoke more frequently than that you get hit with an incurable addiction wound, where you have to keep smoking or suffer increasing stat decreases.
It would probably help create a market for it actually, as the addicts have to maintain a steady supply of pipeweed to suppress the debuff.
