Seeing as to how there is no possible way of healing this at this very moment in time, why not add something like stitches, you can make them with like silk filaments. Then if you want to make it even more complicated, you can add a needle that can be made with glass and a nugget of any metal.

Stitches sound great,

'Needle', made from 1 any metal nugget on an anvil.

'Fine Thread', you get 5 pieces per 1 wool on a yarn spinner. Also doubles as 'Any String.'

'Suture Kit': Heals Fell Slashes, Wretched Gore, Deep Cut (any flesh rending wound)
Requires : 1 Needle, 3 Fine Thread.
Uses: Dexterity & Sewing as quality modifiers.

This would make masochists that use yarn as string upset.

I also think Intelligence should have something to do with any kind of surgery.

And a scar which causes a permanent malus to charisma. Although... some scars are badass maybe you have a 25% chance to increase charisma and a 75% chance to lose one.

Could even have another person be able to do it for you, and can even put in a new first aid skill that decreases the chances of receiving any of these other than maybe bleeding. Another thing can be to stop the bleeding after applying the stitches is to put on a gauze, if not treated with a gauze, the bleeding will continue to take off more HHP until rid of by using the gauze on it.
