Question about maps

Most of the maps I've seen have "trails" of mapped terrain towards the outside. Is this the way the real map looks ingame, or is it because those areas haven't been mapped out yet?

So no one has been to those spots! If so, there's so much terrain unexplored!!!

How does the map update??
