How about something like this:
When you create new character you have 2 options:
normal - wilderness
or start as a child (instead of HS of a character, you give HS of a grown child)
This way if you want an alt at your place you'll have to travel from your spawn place in the wilderness or make and rise a child... Of course it can be also your friend that start as your child
