Any time I've ever had this issue it was after I've skinned the carcass and left it.
I don't know if these have been suggested, but I'm thinking they can be possible causes:
1: After a decay tick, the server code treats the skinned carcass as an untouched one and is somehow corrupted being saved as the wrong object
2: It could be unexpected in the server code that the player skins an animal, leaves it sitting there, and logs out before a decay tick causing the area to be unloaded.
3: Mix of 1 and 2
I'd say it's 3 because I can envision a situation where reloading an area with a skinned carcass that's being treated as an untouched carcass could cause problems when updating decay ticks on said carcass
I have an idea for a way to reproduce the bug, will update with results when I find a subject to test on
Edit: test failed, will try with multiple animals sooner or later
