The placement of the gate and the entrance places them on the other side of the gate. It's absolutely different than what you've described.

I wasn't there myself, but my understanding is that this is false (from Misa). Although you can normally click out of a cave when you would otherwise be obstructed by collision, in this case, the visitor debuff blocked the action, likely because Visitor gates are by their very design made to block all sorts of actions and movement when in combat.
You are able to interact with caves when you have visitor debuff. You are able to exit caves from the spot you entered. They would have been able to leave, until they ran completely through the gate.
This really isn't that different from someone having a high strength character sitting on a cave entrance and pushing you as soon as you run in and all their friends then beating you up. Running into a cave blindly has always been dangerous.
