You attempt to block a melee weapon with your hands, severing them in the process. You are now unable to attack and begin to bleed to death.

Actually, the beginning of the word 'useful' starts with a semi-vowel, which makes 'a useful' correct

Or just read it like 'a /ju:zful/
"An" works in front of spoken vowels :]
