Wretched gore

Make wretched gore rise slowly over time.
Add a special salve, which made of some easy to find leaves/berries
When salve used on gore, it will stop growing for some time, after what this will continue.

Stitch patch can be used only when salve is on gore.

I think this gore can make the players enough punished. I do not think that new player slowly dying without knowing why can be a good mechanism in the game.
