Ive been following all the patches but must have missed this, was perma death reimplemented? If so, could you link the update?

I'm under impression that ability to attack KO'd hearthling has been (and still is) removed, meaning you have to hit them with enough damage when they have little SHP to completely deplete their HHP bar in single hit. Leech trick may work as well (putting hungry leeches on every slot of the victim that suck the remaining HHP out.)

Might work if you have some really high Q leeches, or they have very few HP left.
