There is no form?

After logging in I can not see the form. What happened?

Maybe a rollback knocked off your char data.

Something had to be fixed, so the programmer had to return to an old save of the map. Maybe your character and its data was lost in the process.

I guess I automatically assumed juju had read the 64 page announcement stating that a new world had started.
