Here are some of the forms that you may need to fill out to make your official application (I'm really not sure why this info isn't in the OP ?)
If you've wronged Dis in the past (as I have) you must first fill out a formal apology
After this you can move on to the declaration of intent which should be sent to LadyGoo through PM only (I had to send several before getting a reply)
Should you succeed in getting Goo's attention you will be required to fill out this NDA in regards to any further personal messages sent between the two of you
If you have not previously harmed Dis or LadyGoo but she has responded negatively to your declaration of intent, please refer back to the formal apology document and begin the process over.
Now, persistance is key here, but things can go either way from this point.
If all goes well, you may send this Statement of Gratitude to Dis or to LadyGoo in PM (depending on how well received you were)
If Goo has accepted your letter of intent, apologies, AND you have signed the NDA with Goo concerning your PMs, you may want to consider filling out this final document
But before you do that, we strongly advise you to fill this out and keep it somewhere safe where your loved ones can easily find it
